Create a Transition Plan

Creating a transition plan is an important first step in the transition process. A transition plan can help family members and school team members see the long-term goals for a student. There are many different types of resources out there to help students identify their long-term goals. Below are just a few resources to help families start the process.

Charting the LifeCourse is designed to be used for your own life or for your family members. The framework and tools will help you organize your ideas, vision, and goals, as well as problem-solve, navigate, and advocate for supports.

TransCen, Inc developed the Positive Personal Profile (PPP). The PPP is a simple document outlining an individual’s interests and strengths, used to identify what is important and what the individual aspires to achieve. The focus is on skills and preferences, rather than labels and deficits. This positive approach helps us to determine what the individual can do and loves to do. It also identifies the person’s passions and values, which helps to determine what is meaningful for the individual.

The Student Dream Sheet is an interview format transition assessment to help your student identify what their life looks likes now and what they would like their life to look like in the future.

Charting the LifeCourse created a tool for exploring decision making supports. This tool helps individuals look at what decisions they can make by themselves and which decisions they continue to need support to make. This tool would be a good resource for students looking to live independently or go to college.

The Office of Disability Employment and Policy provides an Independent Living Checklist for individuals who are interested in learning if they are prepared to live independently.
